If you think that it is to late and a Trojan has already been installed on your PC then you can get get software that detects and deletes a wide range of Trojans - like anti-virus software you need to update this software regularly to ensure you are protected against the latest Trojans.

However, perhaps the best way to protect yourself from hackers is to install a firewall - with a firewall you can choose which programs can access the internet - and therefore protect large areas of your PC.
